The phase swap is at around 40% and two of us at ~220 ilvl are enough to kill Kil'jaeden during the cast for the last phase. Just save all cooldowns and bloodlust and everything for that part. If you've got good ilvl and a bursty class you can probably solo it at this ... Not sure about add-on. But that would be a pretty easy weakaura to make.
